Adhering to a Harm Reduction model, the program works with the young adults to create stability and overall wellbeing in their lives. The Position – Case Manager  This position will provide direct support to the tenants to ensure financial supports are in place, access to health and mental health services, food bank services, community supports and access to adult education programs as well as other supports that may be required. The case manager will have a well-defined knowledge of resources available in the community and working relationships within service providers. The case manager will take on the role of a “systems navigator” and will support the tenant by identifying the individual’s needs and create a plan with them to access the needed supports and services. The case manager will be the advocate for the tenants and will assist the tenant to successfully receive the supports they are entitled to.The case managers will play an active role within the program to with facilitating meetings and provide in-house workshops and training. The case managers will also be actively involved and present with tenants within the daily life and routines established within the facility. The case managers will be the on-site support for all tenants seeking information regarding community supports and services. They will be active in providing information and connecting tenants to services as required.The case manager will have experience working within the homeless sector and/or working with youth and young adults at risk of homelessness. THE CLIENT POPULATION: Home Health clients typically have on-going health issues and/or complex personal and/or soical issues, requiring care planning and coordination of approporiate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnessess requiring supports to maintain/promote independence. Evergreen serves the northeast part of the city including the Cedar Cottage, Hastings-Sunrise and Renfrew-Collingwood neighbourhoods. Qualifications Education & ExperienceCurrent practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).Three years (3) recent clinical nursing experience working in acute care with a medical and/or geriatric focus, recent case management or home care nursing experience or an equivalent combination of education, training and experience.Local area travel requires the use of personal vehicle. Valid BC Driver’s License.Knowledge & AbilitiesDemonstrates ability to problem solve using sound judgment in applying critical thinking skills within safe limits of client care.Demonstrated skill in physical assessment and chronic disease management.Demonstrated competency in psychomotor skills required within the designated scope of nursing practice.Comprehensive knowledge of BCCNM standards of practice and guidelines for clinical practitioners.Demonstrated ability to work independently and set priorities for care for a caseload of clients.Comprehensive knowledge of other health disciplines and their role in client care.Demonstrated ability to work collaboratively as a member of a interdisciplinary team.Comprehensive knowledge of adult education principles, methods and tools.Demonstrated ability to use research and methodology in practice.Demonstrated ability to provide leadership and work direction.Demonstrated skill in facilitating groups that include professionals/clients and or clients/family members.Demonstrated ability to teach, demonstrate, coach and facilitate clients and families.Demonstrated ability to effectively communicate orally and in writing.Demonstrated ability to communicate, negotiate and deal effectively with co-workers, physicians, other health care staff, clients and their families, and staff of outside agencies.Demonstrated ability to effectively deal with conflict resolutions.Demonstrated ability to adjust to new or unexpected events.Demonstrated ability in conducting physical, mental, psychosocial/functional environmental assessments.Demonstrated skill in the use of supplies and materials utilized in the provision of community based nursing service.Demonstrated physical ability to perform the duties of the position.Computer literacy to operate a computerized patient care information system and email system.Ability to operate related equipment.
