The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders. The LPN Outreach operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and in conformance with established policies, procedures, standards and philosophies of Fraser Health.
