Join our growing pharmacy team in providing safe and effective medication use to our clinical units at Hamilton Health Sciences.At Hamilton Health Sciences, our pharmacy department plays an essential role in ensuring the safe distribution and service of medications to patients which facilitates successful patient treatment outcomes in compliance with legal, regulatory and accreditation standards.  The management and distribution of our medications is important in ensuring we can consistently deliver the high-quality, safe care that we are known for.  We are growing our pharmacy team and presently seeking a pharmacist to support us.  The pharmacist is a key member of the multidisciplinary healthcare team whose key responsibilities include reviewing and assessing medication orders and collaborating with the patient and healthcare team directly to optimize medication use, identifying, and avoiding medication-related problems to achieve desired patient outcomes. The pharmacist will provide drug information, promote evidence-based/cost effective drug therapy and clinically relevant research and be involved in educational activities for students, families, and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential. Position Responsibilities:• Responsible for the provision of clinical pharmacy services for the purpose of achieving definite outcomes to improve the patient’s quality of life• Review of physician’s orders, creation, and maintenance of patient medication profiles, and may include IV admixture, TPN, pediatric and chemotherapy preparation, drug information, drug therapy monitoring and adverse drug reaction reporting depending on the site• Order entry of medication orders with future transition to computerized physician order entry in EPIC system• Other duties as assigned

Reporting to the Director of Pharmacy Services and functioning as a member of the Pharmacy leadership team, the Pharmacy Site Manager is responsible for providing operational and clinical practice oversight of the site-specific inpatient pharmacy to ensure high quality pharmaceutical care delivery to patients, and compliance with the Ontario College of Pharmacists standards to maintain College accreditation status. Participation in and leading organizational medication system quality and transformation projects is key, as is collaboration with the various stakeholders within the medication system. Areas of accountability include operations management related to staffing and service delivery, human resource management, financial management, quality assurance, leadership and maintenance of professional practice.

The Pharmacy Technician/Assistant processes prescriptions for the pharmacist to check before dispensing to the patient, assists the pharmacist by conducting technical functions in the pharmacy (including inventory control; drug purchasing for the retail pharmacy and other related functions), communicates with patients regarding their prescription needs and refers them to the pharmacist when necessary. In this role, the primary function of the pharmacy technician will be to provide support in the retail pharmacy, however the incumbent will be cross-trained to provide back-up support in the clinical pharmacy as may be required to meet patient needs.

Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.Duties and responsibilities for the Overnight pharmacist will include supporting Hamilton General, Juravinski, St. Peter’s and West Lincoln Hospitals in addition to the Alternate Health Facility:• Review medication orders for safety and appropriateness, and verify orders in a fully electronic computerized prescriberorder entry health information system• Provide drug information and clinical overnight pharmacy support (e.g. pharmacokinetic monitoring, etc)• Responsible for the oversight of drug distribution services in collaboration with pharmacy staff• Effectively communicate with nursing, medical staff, pharmacy team members and management• Other duties as assigned
