Provide or assist as directed, according to client care plan with client vital sign measurement, documenting and communicating the results to team leader. Provide or assist as directed, according to client care plan, with the application of oxygen therapy to include; portable oxygen, oxygen concentrators, and wall mount oxygen. Provide or assist as directed, according to client care plan, with the emptying of closed suction medical systems to include; suction canisters, Hemovac, Blake and Jackson pratt drains, documenting and communicating results to team leader. Provide or assist as directed, according to client care plan, using medication safety principles with the established application of prescribed creams, ointment and/or oil-based lotions. Provide or assist the client with validation and orientation techniques and, provide privacy and maintain individual identity, confidentiality regarding client’s condition and family affairs. Provide or assist in admissions, transfer and discharge of client including care of the body after death. Lists client’s valuables and arranges for their storage on admission and their return on discharge. Promote and maintain proper hand hygiene and infection control practices including the appropriate use of personal protective equipment. Maintain a clean environment including but not limited to, equipment cleaning, changing linen as necessary, replenish supplies in client room; wipes spills to prevent injury and report all accidents and breakages immediately. Order and stock unit supplies (i.e. linen and food). Encourage and assist client’s participation with activity program. Organize and facilitate leisure activities for individuals and/or groups on evenings and weekends. Demonstrates the safe use of medical equipment applicable to client service area. The Home Care Attendant functions in meeting the special and changing needs of clients and assisting them in reaching and maintaining optimum health and independence. RESPONSIBILITIES: Overview: Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times. Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator. Demonstrates knowledge of standards for safety and sanitation including guidelines for hand washing. Reports concerns of knowledge and/or skill deficits to the Supervisor. With additional education and in accordance with regional policies/operational directives/procedures on assignment and delegation, may be assigned the following client care tasks: * Medication Assistance (oral medications, topical medications, transdermal medications, inhaled medications, eye drops/ointments, ear/nose medications, subcutaneous injections) * Enteral feeding (via pump and via gravity) * Blood Glucose monitoring * Ostomy Care * Adjusting oxygen flow rates Records activities and observations as per policy. Assists with meal preparation in accordance with the “Canada Food Guide” or the client specific diet. Organizes work according to client’s needs and the assigned tasks. Reports any changes in the client situation to the Supervisor and/or Case Coordinator. Reports accidents/incidents immediately to the Supervisor or Case Coordinator and/or the client contact. Reports repairs of equipment and physical facilities to the Supervisor and/or Case Coordinator; Maintains client confidentiality. Participates in staff meetings, case conferences, and in-service programs as required. Communicates with co-workers involved to achieve the client-centered goals. Maintains regular reporting relationship with Supervisor. Other duties as assigned.
