The position covers services for patients living in Hants County, referred by their primary care providers who wish to refer their patients for concerns such a Falls, Memory Loss, Polypharmacy, Dementia, Anxiety, Depression, Behavioral and Psychological Symptoms of Dementia, Caregiver Burnout and Capacity Assessments. Referrals may come from the Community (out-patient), in patient ( mainly Hants Community Hospital) and local Nursing Homes. With regards to volume, on average a physician in this role can expect to see approximately 130 new consults a year and many follow ups. In this role you can expect to collaborate with teams that include Continuing Care Coordinators, Social Work and Nursing, Geriatric colleagues and Seniors Mental Health in Halifax for complex cases, out-patient Occupational Therapy and Physical Therapy, Seniors Link Program in Kings County and the Alzheimer’s Society of NS. This position is remuneration through the AFP at $160.00/ per hour, for a maximum of 16 hours per week. There is opportunity to engage in research and/or teaching positions with Dalhousie University, as well as, local simulation based opportunities, should that be of interest. HCR ED is approximately 40 minutes drive from the region's tertiary care pediatric facility, the IWK Health Centre, and the Queen Elizabeth II Health Sciences Centre. HCH services include: 24-hour emergency department inpatient care (28 hospital beds) long-term care facility (32 beds) surgery programs including general and orthopedic surgery several specialty clinics (including obstetrics, gynecology, pediatrics, hematology) mental health team On call services are provided from Halifax and Kentville. About This Opportunity Avon Medical Clinic is a collaborative medical practice located on Gerrish Street in Windsor, NS. There are currently three physicians, a Nurse Practitioner, and a Family Practice Nurse on the team, as well as four amazing front desk staff and an outstanding office manager. This clinic collaborates between all practitioners, supported by a family practice nurse for teaching, injections, well baby visits and prenatal care. Minor procedures are done either in office or at the local hospital, with nurse support. This clinic may also soon have co-located primary care clinic with a duty component.
