Staff will play a key role for the client and to healthcare providers in the system. The team will support the client from hospital back home and will also provide treatment at home as an alternative to hospitalization when appropriate. The acute and community systems will rely on this team to do the very important work of providing comprehensive mental health and substance use care in the client’s home.
