Join our growing pharmacy team in providing safe and effective medication use to our clinical units at Hamilton Health Sciences.At Hamilton Health Sciences, our pharmacy department plays an essential role in ensuring the safe distribution and service of medications to patients which facilitates successful patient treatment outcomes in compliance with legal, regulatory and accreditation standards.  The management and distribution of our medications is important in ensuring we can consistently deliver the high-quality, safe care that we are known for.  We are growing our pharmacy team and presently seeking a pharmacist to support us.  The pharmacist is a key member of the multidisciplinary healthcare team whose key responsibilities include reviewing and assessing medication orders and collaborating with the patient and healthcare team directly to optimize medication use, identifying, and avoiding medication-related problems to achieve desired patient outcomes. The pharmacist will provide drug information, promote evidence-based/cost effective drug therapy and clinically relevant research and be involved in educational activities for students, families, and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.Position Responsibilities:• Responsible for the provision of clinical pharmacy services for the purpose of achieving definite outcomes to improve the patient’s quality of life• Review of physician’s orders, creation, and maintenance of patient medication profiles, and may include IV admixture, TPN, pediatric and chemotherapy preparation, drug information, drug therapy monitoring and adverse drug reaction reporting depending on the site• Order entry of medication orders with future transition to computerized physician order entry in EPIC system• Other duties as assigned
