The Nurse Practitioner (NP) for the Acute Medicine Program at the Hamilton General Hospital will be registered with the College of Nurses of Ontario in the Extended Class-Adult, and will work within the scope of practice of the RN-EC provided by the College. Collaborating with interdisciplinary team members, and using a patient and family centered model of care.  The NP is responsible to: provide direct patient care, including the assessment and treatment of complex medical patients, coordination of care, discharge planning, and the coordination of related community and other resources for this patient population; tutor and mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate, and/or lead programs, presentations, publications, evaluations and other system changes to continue the enhancement of interdisciplinary care teams.  The Nurse Practitioner will work in partnership with General Internal Medicine, consultants, nursing and other members of the health care team to provide comprehensive medical assessment, management and education.

The ICU West is a 16 bed, level 3 tertiary and quaternary unit performing the highest volume of cardiovascular surgical procedures in the province.  Using a multidisciplinary and collaborative team approach, dedicated to the principles of patient-and family-centered care, the Registered Nurse coordinates and evaluates patient care and provides nursing care according to the Standards of Practice of the College of Nurses of Ontario, and Standards of Practice for Critical Care Nurses in Ontario. Our nurses treat highly complex patients using innovative treatment modalities including Extracorporeal Membrane Oxygenation (ECMO), Impella, Intra-aortic Balloon Pump and Continuous Renal Replacement Therapies (CRRT).  The ICU also supports the Critical Care Response Team for the HGH site, bringing highly skilled, specialized, critical care nurses, as members of a multidisciplinary team, to offer support and consultation outside of the ICU.   As a Royal College Accredited training program and through our affiliation with McMaster University, we offer opportunities for learning and conduct world renowned research.

Acute Medicine 8 South is an active adult general internal medicine unit with a 4-bed Step-Down Unit. An interdisciplinary team provides care to an acutely ill, primarily geriatric, patient population, including patients with multi-system chronic illnesses. Using a client-centered approach, the Registered Nurse (RN) provides leadership in the coordination and evaluation of nursing care of patients with multisystem medical conditions awaiting transition to the community setting. The RN works collaboratively with interdisciplinary team members and community agencies to coordinate care and education of patients and their families. Through evidence-based clinical practice and strong leadership in healthcare delivery, the RN sets the standards of excellence in the care of patients at HHS. The RN establishes and implements education plans with and for patients and families and acts as an accessible and knowledgeable resource for patients and their families.

The Registered Practical Nurse provides patient care in accordance with the standards of The College of Nurses of Ontario, nursing standards and policies/procedures at St. Peter’s Hospital and current Gerontological Nursing Practice. Reporting to the Clinical Manager or delegate, the Registered Practical Nurse provides nursing care under the direction of and in collaboration with the RN and/or Case Manager. 

Unit Summary The Surgical Team at the General Hospital provides post-operative care to the critical and adult patient population. In this exciting and dynamic environment, our team supports tertiary and regional programs in cardiac, vascular and neurosciences and trauma providing both inpatient and same day surgery. Position Summary As a member of this specialty unit providing critical services, the PACU Registered Nurse is responsible for the planning, implementation and assessment of the post anesthetic patient care. As a member of the healthcare team, the Registered Practical Nurse will provide high quality nursing care to patients in collaboration with the Registered Nurse on the unit. The RPN will participate in continued education and completion and maintenance of certifications associated with the identified nursing skills and complete learning plans to achieve these goals. The RPN will participate and support the quality initiatives on the unit  to continually improve practice.

An interdisciplinary team provides care to an acutely ill, primarily geriatric patient population, including patients with multi-system chronic illnesses. 5 West utilizes a patient & family-centred, evidence based approach to care and is frequently recognized for its positive impact on patient experience. The 5 West department at HGH takes an innovative approach to our orientation programs. We welcome nurses entering the profession and are committed to ensuing a successful transition from student to nurse. Our 5 West Registered Nurses care for a wide range of patients with complex medical issues. All medicine nurses are set up for success and go through a comprehensive onsite orientation program, led by experienced health care professionals and educators.
