In addition to the Family Health Centre, we run a Seniors Health Centre, a Youth Centre, two Mobile Community Health Buses, one Dental Health Bus, four Housing First Programs, and a Community Food Centre.What we do and why we do it:Pathways to Housing, HomeBase, Prelude, and Abbeydale Place are Housing First Programs through The Alex that support formerly homeless program participants with significant physical and/or mental health issues.  Participants are provided with subsidized housing and rapid access to a myriad of supports to encourage stability, recovery, and improved quality of life.Pathways to Housing is an Assertive Community Treatment (ACT) Team that supports individuals who have psychotic illness and have experienced chronic homelessness.  The full-time Registered Nurse is a key member of the Pathways Team and is responsible for providing medical interventions to support program participants to achieve stability and wellness while reintegrating into community.  The Registered Nurse offers assessment, treatment, education, orientation, referral, urgent response, and triage for program participants.  The Registered Nurse contributes to a wide range of needs and is responsive to ACT Team duties.
