Permanent Job Summary Under the direction of the Director of Primary Care the Nurse Practitioner (NP) respectfully works with diversity including sexual and gender minorities, and across all ethnicities and cultural, spiritual, political, age, ability, family and economic circumstances. Using a culturally safe, client centered, strength based, and harm reduction approach and with an emphasis on providing HIV/sexually transmitted and blood borne infection (STBBI) care, the NP provides competent primary care and episodic same day health services in clinic and outreach settings including community settings, shelters, encampments and client’s homes. As part of the interdisciplinary team, the NP works independently and collaboratively to provide professional, evidence-informed, holistic care within a low barrier clinic environment and in a range of community settings. As part of a primary care team, their care includes assessment, diagnostic testing, diagnosis, clinical intervention and treatment, care planning, prescribing, minor procedures, follow-up, teaching, illness prevention, health promotion and education, and chronic disease management. The focus of this position is for in the moment care. The job is a full-time, 77.50 hours per two-week period (including holiday pay hours).
