The physiotherapist collaborates with interdisciplinary team members in providing Phase 1 rehabilitation and education to patients and families. The physiotherapist functions as an integral member of the Physiotherapy Cardio-Vascular team providing services to 4 West and CCU.The physiotherapist is proactively involved in discharge planning collaborating with interprofessional team members and community agencies.
