The Nurse Practitioner (NP) for the Acute Medicine Program at the Hamilton General Hospital will be registered with the College of Nurses of Ontario in the Extended Class-Adult, and will work within the scope of practice of the RN-EC provided by the College. Collaborating with interdisciplinary team members, and using a patient and family centered model of care.  The NP is responsible to: provide direct patient care, including the assessment and treatment of complex medical patients, coordination of care, discharge planning, and the coordination of related community and other resources for this patient population; tutor and mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate, and/or lead programs, presentations, publications, evaluations and other system changes to continue the enhancement of interdisciplinary care teams.  The Nurse Practitioner will work in partnership with General Internal Medicine, consultants, nursing and other members of the health care team to provide comprehensive medical assessment, management and education.

The ICU West is a 16 bed, level 3 tertiary and quaternary unit performing the highest volume of cardiovascular surgical procedures in the province.  Using a multidisciplinary and collaborative team approach, dedicated to the principles of patient-and family-centered care, the Registered Nurse coordinates and evaluates patient care and provides nursing care according to the Standards of Practice of the College of Nurses of Ontario, and Standards of Practice for Critical Care Nurses in Ontario. Our nurses treat highly complex patients using innovative treatment modalities including Extracorporeal Membrane Oxygenation (ECMO), Impella, Intra-aortic Balloon Pump and Continuous Renal Replacement Therapies (CRRT).  The ICU also supports the Critical Care Response Team for the HGH site, bringing highly skilled, specialized, critical care nurses, as members of a multidisciplinary team, to offer support and consultation outside of the ICU.   As a Royal College Accredited training program and through our affiliation with McMaster University, we offer opportunities for learning and conduct world renowned research.

The Nurse Practitioner (NP) for the Pediatric Weight Management Program at McMaster Children’s Hospital will provide leadership and promote excellence in pediatric family centered nursing care through expert clinical practice, education, consultation and research. The NP would support the clinical activity of the Endocrinologists / Pediatricians and the associated patient care and interventions through the Pediatric Weight Management Program. The program is most focused in outpatient care, although occasional inpatient care occurs. Collaborating with the interdisciplinary team members, and using a patient and family centered model of care, the NP is responsible to: provide direct patient care, coordination of care, discharge planning, and the coordination of related community and other resources for this patient population; tutor and mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings; initiate, participate, and/or lead programs, presentations, publications, evaluations, research initiatives and other system changes to continue the enhancement of interdisciplinary care teams. The NP will work in partnership with General Pediatrics, consultants, nursing and other members of the health care team to provide comprehensive medical assessment, management and education. The NP reports to the Director, Critical Care and Ambulatory Services, McMaster Children's Hospital, and the Chief of Nursing Practice.

The SHF is part of the Community & Population Health Program and approved to operate until March 2024. Registered Practical Nurses (RPNs) recruited for the SHF are hired into permanent positions through the Nursing Resource Team (NRT).  The RPN will work as part of a multidisciplinary team within the SHF. Our SHF nurses are set-up for success and go through a comprehensive on-site orientation program, led by experienced health care professionals and educators. As a member of the multidisciplinary healthcare team, the RPN coordinates and evaluates patient care and provides nursing care according to the standards of practice for RPNs.

Unit Summary The Surgical Team at the General Hospital provides post-operative care to the critical and adult patient population. In this exciting and dynamic environment, our team supports tertiary and regional programs in cardiac, vascular and neurosciences and trauma providing both inpatient and same day surgery. Position Summary As a member of this specialty unit providing critical services, the PACU Registered Nurse is responsible for the planning, implementation and assessment of the post anesthetic patient care. As a member of this interdisciplinary healthcare team, the Registered Practical Nurse provides patient care in accordance with the standards of The College of Nurses of Ontario, nursing standards and policies/procedures at St. Peter's Hospital and current Gerontological Nursing Practice. The Registered Practical Nurse on Medically Complex provides person-centred care and functions within the full scope of practice of the RPN by gaining the knowledge and skills to build upon foundational knowledge to promote the best possible psycho-social, emotional, and physical outcomes for patients on Medically Complex.  The RPN collaborates with the RN using the College of Nurses of Ontario’s three-factor framework, and works closely with other nurses and members of the interdisciplinary team, and may assign tasks and elements of patient care to unregulated care providers. The RPN is supported by the Clinical Manager and other nurses including Registered Nurses, Nursing Case Managers, Nurse Practitioner, Clinical Nurse Specialist, and Education and Development Clinician.
