The Nurse Practitioner (NP) for the Pediatric Weight Management Program at McMaster Children’s Hospital will provide leadership and promote excellence in pediatric family centered nursing care through expert clinical practice, education, consultation and research. Nurse Practitioner

As a member of the interdisciplinary Health Care Team, the Acute Care Nurse Practitioner (RN-EC) uses clinical expertise and in-depth knowledge of children, adolescents and families, with a focus on eating disorder related illnesses, processes and issues, to plan patient care and evaluate clinical practice across the continuum of care for children and adolescents and their families. This also includes the application of knowledge and treatment interventions for co-morbid mental health presentations. The RN-EC will provide clinical leadership and promote excellence in the Pediatric Eating Disorder Program including family centred nursing care through advanced clinical practice, interprofessional educational support, consultation, leadership and research. The RN-EC role will be to provide advanced practice nursing care for with eating disorder and health related issues The RN-EC will support families with children who are part of the Eating Disorders Program. The RN-EC will be instrumental in the delivery of care including the identification, sharing and implementation of best practices where applicable. The RN-EC will facilitate the enhancement of the delivery of inter-professional care. The RN-EC will work collaboratively with the team to develop and implement a plan of care. The RN-EC will be involved in the identification and delivery of interventions including group psycho education, individual and group cognitive behavioral therapy and family therapy. The RN-EC will collaborate with the physician team in the medical management of patients within the eating disorder program. Included in those responsibilities will be the assessment and monitoring of interventions, including pharmacologic therapy. The RN-EC will also participate as a community liaison and patient advocate. Proven effective collaboration and communication skill with health care teams is essential. The RN-EC will bring evidence-based practice in the field of pediatric eating disorders and will incorporate research and evidence into nursing and inter-professional practice and policy. The RN-EC will work collaboratively with the team to develop and implement a plan of intervention, which addresses physical and mental health and wellness. The RN-EC will liaise with the patient's referring physician and with other treatment programs to ensure that the patient receives appropriate medical care. Utilizing advanced nursing knowledge, skill, clinical reasoning, problem solving and critical thinking, all duties and responsibilities will follow the RN-EC Standards of Practice set by the College of Nurses of Ontario, and Code of Conduct set by Hamilton Health Sciences. Oncology Day Services provides integrated and outpatient care for individuals with hematological and oncological disorders in an ambulatory setting. Within the umbrella of ODS, is the Stem Cell/Bone Marrow Transplant and Apheresis Programs. The primary therapies supported in ODS are blood product infusion, medication infusion, patient assessment, stem cell harvests, post stem cell/bone marrow transplant follow up clinic and chemotherapy administration. As a member of the interdisciplinary team, the Registered Nurse evaluates and provides nursing care to meet patient’s needs, and collaborates with the team to provide holistic and evidences based care to our patient population. The Nurse Practitioner (NP) – Addictions, is registered with the College of Nurses of Ontario in the Extended Class. Reporting to the Director of Community & Population Health Services (CPHS), the NP will be instrumental in working with Program leadership and stakeholders to develop and execute strategic plans, and to advance best practices for operational excellence and sustainability. The NP will: • Work in partnership with physicians, nurses and other members of the health care team ensuring comprehensive assessment, management and education of patients; • Provide care to patients experiencing substance use disorders and/or mental health conditions that is recovery oriented, trauma-informed, utilizes principles of harm reduction and addresses social determinants of health;  • Utilize evidence‐based interventions, including withdrawal and other medication management, to meet the needs of a diverse range of patients with substance use disorders; • Act as a resource providing substance use‐related clinical advice to patients and their families, health and other professionals, and community partners; • Coordinates seamless discharge from hospital to community; • Act as an important communication link between patients and other service providers (e.g. primary care and other community partners) to ensure effective communication and the timely sharing of information necessary to promote the health and wellness of patients; • Initiate, participate, and/or lead initiatives including program development, evaluation, and research.
