Nurse Practitioner - Nursing Led Outreach Team - Regular part-time 2023-6368

We currently have a Regular Part-Time (0.5 FTE) opportunity for a Nurse Practitioner in the Nurse-Led Outreach Team (NLOT).   Hours of Work Weekdays & Weekends: Days; 8 hours; Flexibility in hours will be required.   This Nurse Practitioner role in the Nurse-led outreach team (NLOT) is primarily responsible for providing clinical care to support residents living in Long-Term Care (LTC) homes. The Nurse Practitioner (NP) is part of the Sunnybrook North Toronto Ontario Health Team, and aligns with the Infection Control Hub and LTC+ programs. The NP works in collaboration and consultation with physicians, navigators, coordinators, and other members of the inter-professional health care team to prevent avoidable ED visits and hospitalizations, and to support transitions to and from hospital, when needed. The NP will act as a link between the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision.
