We currently have a Regular Part-Time (0.5 FTE) opportunity for a Nurse Practitioner in the Nurse-Led Outreach Team (NLOT).   Hours of Work Weekdays & Weekends: Days; 8 hours; Flexibility in hours will be required.   This Nurse Practitioner role in the Nurse-led outreach team (NLOT) is primarily responsible for providing clinical care to support residents living in Long-Term Care (LTC) homes. The Nurse Practitioner (NP) is part of the Sunnybrook North Toronto Ontario Health Team, and aligns with the Infection Control Hub and LTC+ programs. The NP works in collaboration and consultation with physicians, navigators, coordinators, and other members of the inter-professional health care team to prevent avoidable ED visits and hospitalizations, and to support transitions to and from hospital, when needed. The NP will act as a link between the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision. If yes, we invite you to read the details below then apply.     Hourly Rate Range: $61.51-$76.82   Reporting Relationship: Manager, Ambulatory Medical Clinics   Responsibilities: Development and ongoing evaluation of the Stroke Prevention clinic Responsible for the day to day operations of the clinic Making service delivery/program changes in response to volumes and client needs Promoting stroke prevention best practices in the care of client that have experienced a suspected transient ischemic attack (TIA), minor non-disabling stroke, and/or have vascular risk factors. Triaging urgency of referrals using professional judgment and coordinating diagnostic and laboratory testing that are within the RN (EC) scope of practice by utilizing medical directives.  Provide pharmacological interventions that include prescribing medications as per RN (EC) scope of practice and by implementation of medical directives. Initiate, renew, titrate and discontinue pharmacotherapy as appropriate to ensure vascular risk factors for stroke are well managed meeting stroke prevention guideline targets.  Will evaluate outcomes of individual patient care in collaboration with the interdisciplinary team.  In keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker. Utilizes expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client.  Work in a manner to ensure safety to oneself, clients and their families, colleagues and the work environment. The current emphasis of this role is direct patient care, including the daily clinical management of a defined group of cardiology patients and the coordination of interdisciplinary patient care on D3. In addition, the Nurse Practitioner collaborates with the Patient Care Manager, Clinical Educator and Advanced Practice Nurse to enhance nursing practice through the development, implementation and evaluation of best practices. The Complex Out-Patient Antibiotic Treatment (COPAT) Clinic is an innovative Nurse Practitioner led clinic launching at Sunnybrook Health Sciences Centre to offload occupancy pressures in the Emergency Department (ED) and inpatient wards, by providing safe, effective and efficient outpatient care for patients on oral or intravenous antibiotic treatment for common and serious infectious diseases under the supervision of Sunnybrook Infectious Diseases division. Patients seen in the clinic will be Infectious Diseases patients referred from the SCOPE program, patients discharged from the wards, as well as those requiring urgent follow-up from the Emergency Department.   The Nurse Practitioner (NP) is an advanced practice role designed to contribute to the quality of patient care through practice, research and education.  The current emphasis of this role is direct patient care, including the daily clinical management of a defined group of Infectious Diseases patients in both outpatient and inpatient settings. Collaborates with Infectious Diseases physicians, Nurse Practitioners, Patient Care Managers, all members of the inter-professional clinical team, NP Professional Practice Leader as well as those of the various consult services and Nursing to enhance and improve nursing practice.    As a Nurse Practitioner leading the COPAT Clinic the NP will review patients with Infectious Diseases physicians, see the rapid new assessment patients from Emergency Department/inpatients and outpatient referrals from SCOPE program, as well as existing COPAT patients with urgent complications requiring in person assessment. The NP will manage complex wounds and administer first dose antimicrobials and provide required management of central venous access devices (e.g. PICC lines, ports). Centre for Addiction and Mental Health - CAMH, Toronto, ON
The Acute Care Program is currently seeking a full-time, temporary (until November 22, 2024), Registered Nurses to work in COMPASS (Concurrent Outpatient Medical Psychosocial Addiction Support Service). COMPASS is an integrated research/clinical service incorporating medical treatment for the ill effects of addiction, including medical and non-medical treatment for opioid dependence, alcohol problems, outpatient withdrawal management (day detox) as well as the treatment of clients experiencing chronic pain. In this role, you will be responsible for conducting assessments and contributing to treatment plans that will involve liaising with other services, community agencies, and practitioners to coordinate and deliver care for clients in an efficient and safe manner, as a member of the inter-professional team. You will provide client-centred psychiatric, addictions and general nursing care and ensure the security and safety of the care environment. You will practice in accordance with CNO standards, with CAMH Policies and the CAMH Practice Model, and the Canadian Standards of Psychiatric and Mental Health Nursing Practice (CFMHN). As a member of the inter-professional team you will provide group and individual therapeutic interventions as well as actively engage in interdisciplinary and cross-program collaboration. In this fast-paced environment, you will be expected to utilize critical thinking, and a solution focused customer service approach to direct client care. You will be able to work with clients from diverse populations; contribute to program development; participate in clinical supervision; and provide education support and supervision to students. You will actively participate and support the research activities of the program, and engage in research training to achieve competence in this area. Your skills include the ability to make behavioural observations, mental status and physical assessments, monitoring patient’s response to pharmaceutical and care interventions, documentation and ongoing communication with other team members. You will have sound knowledge of pain assessment and management modalities, crisis intervention, addiction nursing, concurrent disorders, group facilitation, recovery oriented care, and trauma-informed care. Your strengths will include an ability to perform assessments, develop, implement and evaluate inter-professional client-centered care plans within an addiction medicine framework. You will possess excellent communication and interpersonal skills to enable you to develop the Nurse-Client relationship and work within a dynamic collaborative and diverse inter-professional team. Effective leadership, decision-making, organizational and problem solving skills are required. You have demonstrated ability to intervene effectively in clinical situations requiring the prevention and management of disturbed behaviour. Experience in Addiction Medicine nursing is required .Previous experience in case management and clinical assessment is required. The ability to work with clients of diverse cultural and social backgrounds within an Addiction Medicine framework is necessary. Computer literacy is required. Bilingualism (French/English)and/or proficiency in a second language would be an asset.
