POSITION SUMMARY: Reporting to both the Manager of UHN’s Connected Care Hub and the Director of UHN@Home, the Nurse Practitioner, as a member of the Program, provides patient-centered care, to a selected group of patients as they access care within University Health Network and with partner organizations, as required. The successful candidate will conduct in person and virtual clinical care to patients from both the Hub and Integrated Care Program. They will work within a dynamic environment and contribute to building the role within a new program. The role of the Nurse Practitioner is new for the Integrated Care Program and we are looking for an enthusiastic candidate to work with us to define and shape this part of the role and how the team works together to support high quality patient care. The Nurse Practitioner will work with the Hub Nurse Practitioners and Integrated Care Leads to deliver care to identified patients. They will also use that information with program leadership and project management support to develop referral and escalation processes. The Nurse Practitioner is expected to demonstrate autonomy while assuming and having sole accountability and the authority to independently communicate a diagnosis, prescribe drugs, order a range of diagnostic tests, make referrals, sign-off Medical Treatment Orders, and provide some nursing care as needed for an outpatient client population of the Connected Care Hub and Integrated Care Program. As a nursing leader, the Nurse Practitioner is expected to provide clinical care, research and education. Each Nurse Practitioner will be responsible for collaborative practice within a health care team.
