We currently have a Regular Part-Time (0.5 FTE) opportunity for a Nurse Practitioner in the Nurse-Led Outreach Team (NLOT).   Hours of Work Weekdays & Weekends: Days; 8 hours; Flexibility in hours will be required.   This Nurse Practitioner role in the Nurse-led outreach team (NLOT) is primarily responsible for providing clinical care to support residents living in Long-Term Care (LTC) homes. The Nurse Practitioner (NP) is part of the Sunnybrook North Toronto Ontario Health Team, and aligns with the Infection Control Hub and LTC+ programs. The NP works in collaboration and consultation with physicians, navigators, coordinators, and other members of the inter-professional health care team to prevent avoidable ED visits and hospitalizations, and to support transitions to and from hospital, when needed. The NP will act as a link between the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision. The NP will also support NLOT program development and direction that is agile and iterative, reflective of the dynamic nature of resident care need as well as evolving Ontario Health and Ministry of Long-Term Care priorities. SPEP program participants will have an opportunity to engage in high-fidelity simulation, weekly educational in services, and career coaching exercises. Limitations: The Clinical Extern may not Initiate the 5 Controlled Acts authorized to nursing within the Regulated Health Professions Act (RHPA) of Ontario.
